How to get marriage registration form


  • Marriage registration form is available in the DC office of your district.
  • One can collect the marriage registration form by approaching the reception counter of application
  • You should approach divisional magistrate of your district along with your application form with attached documents.
  • You should accompany with your spouse for the verification process.

Required documents for marriage registration

  • Marriage registration form alone is not sufficient enough to make the registration, you should submit the following documents to complete the process of registration
  • A copy of hindu application form duly signed by married couple should be submitted in case of hindu marriage.
  • A copy of special marriage application form duly signed by married couple should be submitted in case of special marriage.
  • A copy of birth proof which would include your passport copy or matriculation certificate copy should be submitted.
  • Address proof of both man and wife should be submitted.
  • An affidavit which states the date, time and place of marriage and marital status of the person at the time of the marriage also must be submitted
  • Rs10 as the fee of marriage registration form in case of hindu marriage should be paid at the cash counter.
  • Rs15 as the fee of marriage registration form in case of special marriage should be paid at the cash counter.

  • After the submission of marriage registration form and all other required document your application form will go to the progress state.
  • After the verification of the application form the couple will be summoned by the authority for the final confirmation.
  • After the confirmation process you will be issued a marriage certificate which declares a couple as married and registered under Indian court of law legally,
