  5. Stamp Paper Rs. 50.00 for VAT Surtees Rs. 50.00 for CST Surtees Rs. 15.00 for Affidavit.
  6. Application in VAT 1 and Form A
  7. Attested copy of certificate of incorporation, moa and aoa in the case of companies. In the case of partnership firm certified copy of partnership deed
  8. Copy of resolution for opening of sales depot at _______________
  9. Copy of resolution authorising any person to apply and obtain registration certificate both under the vat act and cst act.
  1. Power of attorney in favour of person authorized under the resolution again to apply and obtain registration certificate and also obtain ‘c’ forms, ‘f’ forms, ‘e-1’ forms or any other declaration forms being issued by the excise and taxation department.
  2. List of directors with complete addresses & phone no., i.d. Proof & date of birth of directors alongwith details of immovable property.
  3. List of branches and their sales tax nos if any.
  4. Attested copy of last balance sheet.
  5. Import bill [any purchase bill from outside the state]
  6. Court fee of rs. 25/- and rs.2000/- to be deposited in treasury under the cst act & the vat act.
  7. Bank account number and its address.
  8. List of goods.
  9. 18. Particulars of authorised representative i.e. Name, designation, address, phone no.
